Get official wheel group membership form sorted for bird

Bird apparently filled out an old version of the UCC group membership form as part of getting on-boarded to wheel, and the form is apparently still just sitting there next to the 3D printer.

As such, this will need to be queried with Committee, and possibly a new form filled.
